CHILD SAFE – Code of Conduct Acceptance Declaration<\/a><\/span><\/h3>\n

We require that members of the Committee<\/strong> and each person appointed to the role of Age Manager \/ Deputy Age Manager<\/strong> read our Child Safe – Code of Conduct<\/strong>\u00a0(located on the Child Safety<\/a><\/span> page of this website) and commit to abide by it.\u00a0 To signify that you have read and accept this document, please click on the heading above and provide your personal<\/em> Email Address, Name and Role.<\/p>\n

WWC Declaration<\/a><\/span><\/strong><\/h3>\n

All persons who will be attending Little Athletics and assisting at our meets must<\/strong> provide a completed WWC Declaration form(s) before they can assist at Little Athletics.\u00a0 \u00a0Provision of this form by a parent\/guardian is also a pre-requisite for child(ren) to complete the registration process, collect their registration pack and to participate at St George.<\/em><\/p>\n

Certain roles (eg. Committee Member, Age Managers, etc) are required to provide a WWC Number (which can be obtained from:\u00a0 Service NSW WWC Check<\/a><\/span> ).\u00a0 \u00a0 However, in most cases, “Close Relatives” (<\/em>Parent \/Legal Guardian, Grandparent, Sibling, Aunt, Uncle, Nephew or Niece) of a child registered at St George Little Athletics will not need to provide a WWC number.<\/p>\n

Parent Duty Commitment<\/a><\/span><\/strong><\/h3>\n

This form needs to be filled out (by either parent) once on behalf of the family.\u00a0 \u00a0Please read it carefully.\u00a0 \u00a0It records commitments you are making to assist the delivery of the Little Athletics program at St George.\u00a0 Completing this form is mandatory<\/strong> & a pre-requisite for your child(ren) to complete the registration process, collect their registration pack and to participate at St George.<\/em><\/p>\n

Incident Report<\/a><\/span><\/strong><\/h3>\n

Use this form to report an “Incident<\/strong>” at a St George Little Athletics event (eg. Friday ‘meet’ or Training session) which you believe the Committee needs to know about.\u00a0 Incidents include:\u00a0 Equipment Damage or Theft, Bad Behaviour, Safety Hazard, Injury, or a serious matter concerning Child Safety\/Wellbeing.<\/p>\n

Feedback<\/a><\/span><\/h3>\n

Use this form to provide feedback (positive or negative) on any aspect of the Centre or its programs.<\/p>\n

Application for Participation Credit<\/a><\/span><\/strong><\/h3>\n

To satisfy the \u201cFriday Night Participation Criterion<\/strong>\u201d, an athlete, upon completion of the season, must have attempted at least a minimum percentage of the Events offered to\u00a0his\/her Age Group across the season, as specified in the\u00a0\u00a0 Awards Program<\/a><\/span><\/p>\n

From time to time, athletes will want to attend athletics carnivals<\/strong> or other athletics-specific events<\/strong> held in locations and\/or at times which would prevent\u00a0the athlete from participating in our weekly Friday competition program. In certain cases, we will credit the athlete\u2019s STGLAC participation measure so that\u00a0attendance at the carnival does not prejudice their club participation measure.\u00a0 Use this form to request such a credit.<\/p>\n

Note:\u00a0 This form is only beneficial if the athlete is at risk of not meeting the athlete participation target.<\/em><\/p>\n

Application for Financial Support<\/strong><\/a><\/span><\/h3>\n

We offer an ‘Athlete Support Scheme’, whereby athletes who have earned entry into specific major carnivals or other prestigious athletics events can be supported by way of a financial payment.\u00a0 \u00a0The details of the scheme including the eligible carnivals\/events, the amounts payable and applicable criteria are currently under review.<\/p>\n
